Job summary

Midland Care Connection is a not-for-profit, community-based organization serving NE Kansas with hospice, home health, and PACE programs. The Physical Therapist Assistant supports independence by guiding therapeutic activities and mobility training in PACE centers, homes, or community settings.

Under a licensed PT, the PTA helps with exercises, safety, and fall prevention while documenting progress and coordinating with a multidisciplinary team to maximize quality of life for older adults in our

Qualifications

• Associate degree from an accredited PTA program.
• Current Kansas PTA licensure or eligibility.
• Knowledge of physical therapy and rehabilitation principles.
• Ability to work under supervision of a licensed physical therapist.
• Ability to communicate effectively with participants and care team.
• Ability to document accurately and timely.
• Ability to work as part of an interdisciplinary team.
• CPR/BLS certification required.

Responsibilities

• Provide therapeutic exercises and functional activities to maintain or improve strength, balance, and mobility.
• Assist participants with mobility and transfers using safe techniques.
• Monitor progress and communicate changes to the supervising PT.
• Educate participants and caregivers on exercises and fall prevention.
• Adhere to care plans within PACE center, home, or community settings.
