Job summary

Midland Care Connection in Kansas seeks a Licensed Physical Therapist to join its PACE program, serving older adults in home and community settings.

You will work on an interdisciplinary team to assess mobility, design individualized treatment plans, and promote independence, fall prevention, and quality of life for participants enrolled in PACE.

Qualifications

  • Licensed Physical Therapist in Kansas (KS)
  • Graduation from an accredited physical therapy program
  • Experience with older adults or geriatric populations
  • Knowledge of rehabilitation principles, mobility training, and fall prevention
  • Ability to perform physical assessments and develop individualized treatment plans
  • Strong communication and documentation skills
  • Ability to work within an interdisciplinary healthcare team
  • CPR certification (or ability to obtain)

Responsibilities

  • Assess participants' physical abilities and develop individualized treatment plans.
  • Provide therapeutic exercises and mobility training.
  • Monitor progress and adjust care as needed.
  • Educate participants and caregivers on safe movement, fall prevention, and home strategies.
  • Collaborate with doctors, nurses, occupational therapists, and other team members to coordinate care.
