Job summary

Midland Care Connection seeks a licensed Physical Therapist to join the PACE program, helping older adults stay independent at home and in the community. You will work with an interdisciplinary team to assess needs, design treatment plans, and provide rehabilitative care tailored to each participant.

You will educate participants and caregivers, monitor progress, and coordinate with physicians, nurses, and other professionals to optimize quality of life within the PACE framework.

Qualifications

• Licensed Physical Therapist (PT) in Kansas.
• Graduation from an accredited physical therapy program.
• Experience with older adults or geriatric populations.
• Knowledge of rehabilitation principles, mobility training, and fall prevention.
• Ability to develop individualized treatment plans.
• Strong communication and documentation skills.
• Interdisciplinary team collaboration.
• CPR certification (or ability to obtain).

Responsibilities

• Assess participants' physical abilities and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Coordinate care with doctors, nurses, occupational therapists, and other team members.
