Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health admissions and SOC visits across Monmouth, Middlesex, Union, and Somerset Counties, NJ. You will welcome new clients, perform initial assessments, and set up individualized plans of care with our interdisciplinary team.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Responsibilities

• Perform comprehensive OASIS Start of Care (SOC) admissions visits in Monmouth, Middlesex, Union, and Somerset Counties.
• Conduct thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care with physicians, clients, and care teams.
• Coordinate nursing, therapy, social work, and other home health services.
• Educate clients and caregivers on diagnoses, treatments, and home health plans.
• Document assessments and care coordination in BAYADA EMR accurately.
• Ensure smooth referral-to-admission transition and ongoing home health services.
