Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for home health visits in New Jersey. The RN will perform Start of Care visits, complete comprehensive assessments, and develop plans of care for adults and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties.

The role offers autonomy in the field, collaboration with an interdisciplinary team, and opportunities for professional development within BAYADA’s UCV office.

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.

Responsibilities

• Complete comprehensive OASIS SOC admissions visits for home health clients in the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation and safety assessments.
• Develop individualized plans of care with physicians, clients, caregivers, and interdisciplinary team.
• Identify client needs and coordinate appropriate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care reflects client needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate clinical findings and changes in condition to physicians and BAYADA care team.
• Ensure timely completion of OASIS assessments and admission documentation.
• Accurately document assessments, interventions, education, and care coordination in BAYADA EMR.
• Assist in smooth transition from referral to admission and ongoing home health services.
