Job summary

Mercy Medical Center Home Health and Hospice seeks a licensed Physical Therapist Assistant to provide skilled in-home therapy across Canton, OH and surrounding areas. You will implement plan of care, document patient progress, and educate families, with travel required and a Monday–Friday schedule.

The role requires an active PTA license, at least one year of experience, and strong communication with patients and care teams. Mileage reimbursement is provided.

Qualifications

• Current PTA license in the state of employment.
• Minimum of one year of PTA experience; acute/home health settings preferred.
• Strong communication with patients, families, and care team.
• Accurate, timely documentation per state and agency guidelines.

Responsibilities

• Provide skilled PTA services in patients' homes as per Physical Therapist plan of care.
• Observe, document, and report patient progress and responses to therapy.
• Perform PT-related procedures under supervision and educate patients and families on home programs.
• Complete clinical documentation within agency and Medicare guidelines; participate in case conferences as needed.
