Qualifications

• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as rehab consultant.
• Lead clinical care in SNF settings with interdisciplinary team.
• Collaborate with PCPs on management of post-acute conditions.
• Engage in culture building and ongoing education.
• Attend multidisciplinary meetings and bridge therapy and medicine.
