Job summary

21st Century Home Health Services is seeking a full-time Occupational Therapist to visit patients in Alameda County, Oakland, and surrounding areas. You will evaluate needs, address home safety, wheelchair seating, pain management, falls prevention, and chronic disease management while collaborating with an interdisciplinary team.

The role requires an unrestricted CA OT license, BLS/CPR, and a CA driver’s license. Knowledge of local resources and prior home health experience are highly valued.

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Experience home health care is highly preferred.

Responsibilities

• Evaluate and address home health needs to help patients live safely at home.
• Address home safety, medication management, and wheelchair seating/positioning.
• Manage pain, falls prevention, and chronic disease care; train family caregivers.
• Serve as educator, patient advocate, and member of the interdisciplinary care team.
