Job summary

Medrina is expanding in Jacksonville, FL, seeking a Nurse Practitioner for a Skilled Nursing Facility role focused on post-acute and rehabilitative care. You will work with a lead provider to manage orthopedic recovery, stroke care, and pain management in a collaborative setting.

Ideal candidates have an active Florida NP license, a DEA license, and at least 1 year of NP experience in SNF/geriatrics or rehab. This full-time, on-site position offers mentorship and strong administrative support.

Qualifications

• Active Nurse Practitioner license in Florida.
• Active DEA license or ability to obtain prior to start.
• Minimum 1 year of NP clinical experience (SNF/geriatrics/rehab preferred).

Responsibilities

• Provide comprehensive medical care to SNF patients, including assessments and treatment planning.
• Manage orthopedic rehab, post-stroke, and chronic pain cases.
• Ensure accurate and timely EMR documentation.
