Qualifications

• Active NP license in Minnesota and Wisconsin.
• Active DEA license (or ability to obtain prior to start).
• Minimum 1 year of clinical experience as an NP (SNF, geriatrics, internal medicine, or rehab experience strongly preferred).
• Experience managing adult/geriatric patient populations.
• Strong clinical decision-making and time management skills.
• Comfortable working independently in an SNF setting.

Responsibilities

• Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning.
• Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions.
• Ensure accurate and timely documentation in EMR.
