Job summary

Optum is seeking Senior Community Care clinicians to provide longitudinal, autonomous care to medically complex patients in skilled nursing facilities across Lane County. You will lead clinical decisions with strong interdisciplinary support, managing a dedicated patient panel in a value-based model.

This full-time field-based role emphasizes independence, quality care, and relationships with patients, caregivers and facility teams, with competitive incentives and relocation options.

Qualifications

• Active NP license in home state or ability to obtain by start date.
• Board certification via AANP/ANCC or NCCPA depending on role.
• DEA licensure/prescriptive authority active or obtainable post-hire.
• Ability to gain a collaborative practice agreement where required.
• Must be able to move 30-pound bags, navigate stairs and varied dwelling conditions.

Responsibilities

• Conduct comprehensive patient assessments across multiple SNFs.
• Manage acute and chronic conditions with tests, diagnostics and interventions.
• Oversee short-term sub-acute rehab episodes for members.
• Develop and manage complex treatment plans.
• Lead advanced illness and goals-of-care discussions with patients and families.
• Collaborate with interdisciplinary teams, facilities and caregivers.
• Educate patients and caregivers on care plans and medication use.
• Ensure accurate clinical documentation.
