Job summary

Ennoble Care is seeking a Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in the Philadelphia, PA region. The role is full-time, with supervision by a physician and emphasis on chronic disease management.

The position offers a $3,000 sign-on bonus and a competitive compensation package, plus comprehensive benefits and the autonomy to care for patients in their homes.

Qualifications

• Board Certified NP or PA with PA licensure in PA.
• Strong clinical skills to assess, diagnose and treat.
• Positive demeanor and excellent people skills.
• Ability to solve problems independently in a home-based setting.
• 1–2 years in primary care with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care to homebound patients near Philadelphia, PA.
• Perform examinations, update histories, and develop care plans.
• Communicate effectively with office staff across platforms.
• Deliver patient-centered care to a panel of patients.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients and families; coordinate with caregivers.
• Order necessary diagnostics and prescribe medications and interventions.
