Job summary

Optum Home & Community Care is expanding its Senior Community Care team to support SNF transitions across nursing homes, senior housing, and assisted living settings. This APC role emphasizes assessment, documentation, care coordination, and a focus on improving patient outcomes with CMS guidance.

You will conduct one-time health assessments, educate patients and families, and communicate findings to attending providers while maintaining independence and impact without ongoing treatment

Qualifications

• Active NP/PA certification and state license without disciplinary actions.
• Ability to obtain collaborative practice agreement where required.
• Reliable transportation and valid driver's license for site travel.
• Available to work a minimum of 10 hours per week.
• Proficient documentation in electronic medical records.

Responsibilities

• Perform comprehensive one-time health assessments per CMS/HC regulations.
• Ensure ICD-10 diagnoses are coded and documented accurately.
• Address quality elements and complete medication review.
• Travel between care sites as required and coordinate with team.
• Educate patients/families and relay findings to attending providers.
