Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care team to deliver integrated clinical care in nursing homes, senior housing, and assisted living settings. The Advanced Practice Clinician will coordinate care for high-risk plan members, working with an interdisciplinary team to ensure comprehensive assessments, documentation, and compassionate patient education.

Qualifications

• Active NP or PA certification with valid state licensure
• DEA licensure or authority to prescribe (where required)
• Willingness to obtain collaborative practice agreement if state requires
• Experience in geriatric, long-term or home care is preferred
• Ability to navigate varied care settings and interdisciplinary teams

Responsibilities

• Conduct comprehensive assessments: medical history, medication reconciliation, vitals, physical exam
• Support members during short-term sub-acute rehabilitation episodes
• Ensure accurate and complete documentation including ICD-10
• Communicate and collaborate with interdisciplinary care teams
• Conduct advanced illness and advanced care planning conversations
• Provide counseling and education to patients and caregivers
