Job summary

UnitedHealth Group is hiring an Advanced Practice Clinician to conduct a comprehensive, one-time health assessment for SNF transitions. The APC collaborates with the case management team, documents conditions, reviews medications, and supports care coordination to improve patient outcomes.

The role emphasizes autonomy, accurate coding and documentation, and effective communication with providers and facility staff. Sign-on bonus available and travel is required.

Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license

Responsibilities

• Perform comprehensive assessments and document findings following CMS regulations
• Ensure diagnoses are ICD-10 coded and documented to support the visit
• Independent, one-time health assessment with emphasis on documentation, coding accuracy, and STAR/HEDIS measures
• May travel between care sites and coordinate with the case management team
• Educate patients and families and communicate findings to attending providers
