Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to provide high‑quality care to high‑risk members in home and community settings. You will perform comprehensive assessments, coordinate sub‑acute rehabilitation, and maintain accurate ICD‑10 documentation.

This role involves collaborative practice with an interdisciplinary team, advanced illness discussions, and patient/caregiver education to support a seamless health journey across care settings.

Qualificações

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs; active, unrestricted licensure where you practice.
• Active and unrestricted driver's license and access to reliable transportation.

Responsabilidades

• Conduct comprehensive patient assessments including history, meds, vitals, and exams.
• Support members during short-term sub-acute rehab episodes.
• Ensure complete documentation including ICD-10 coding.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide counseling and education to patients and caregivers.
