Job summary

Rehab Without Walls Neuro Rehabilitation in Farmington, ME is seeking a Physical Therapist (PT) specializing in neuro home and community care. This role offers true one-on-one neuro rehab in patients' homes and community settings with high autonomy and a flexible schedule.

The PT will assess, treat, and educate patients recovering from neurological events, providing functional outcomes in real environments. Local coverage and manageable caseloads are emphasized.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current CPR/BLS certification as required by state regulations.
• Minimum one year of Physical Therapy work experience (preferred).
• Knowledge of neuro rehabilitation techniques and patient safety requirements.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Regularly reassess treatment effectiveness and adjust plans as needed.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient progress to the treatment team and family, and collaborate with other disciplines.
