Qualifications

• Master's degree from an accredited NP program.
• National Board Certification: FNP, AGNP, or ANP.
• Current DEA Registration (or ability to obtain).
• Current Professional Liability Insurance.
• Valid Florida Driver's License and reliable transportation.

Responsibilities

• Perform comprehensive in-home health assessments.
• Diagnose and treat acute and chronic medical conditions.
• Develop individualized treatment and care plans.
• Order, review, and interpret laboratory and diagnostic testing.
• Prescribe medications and monitor treatment effectiveness.
• Educate patients and caregivers regarding disease management and preventive care.
• Coordinate care with physicians, specialists, home health agencies, and interdisciplinary teams.
• Maintain accurate and timely electronic medical records documentation.
• Support preventative care and population health initiatives.
