Qualifications

• Master's degree from an accredited NP program.
• National Board Certification: FNP, AGNP, or ANP.
• Current DEA Registration (or ability to obtain).

Responsibilities

• Perform comprehensive in-home health assessments.

Position Overview

As a Mobile Medical Nurse Practitioner, you will provide primary care services to patients in their homes throughout the Greater Jacksonville and Duval County, FL area.

This role is ideal for experienced Nurse Practitioners who enjoy working independently, managing chronic conditions, and developing long-term relationships with patients in a community-based setting.

• Perform comprehensive in-home health assessments

  • Adult-Gerontology Nurse Practitioner (AGNP)

• Current DEA Registration (or ability to obtain)
• Current Professional Liability Insurance
• Valid Florida Driver's License and reliable transportation
