Responsibilities

• Form bonds with residents, families, and physicians to create care plans.
• Coach coworkers to monitor and support residents' needs with urgency.
• Round with physicians, oversee orders, and monitor medications for quality care.
• Educate residents and families about health issues relevant to the care plan.
