Job summary

Country Meadows Retirement Communities in York, PA is seeking an experienced LPN to join our caring team. You will use your assessment skills to manage resident care, build bonds with residents and families, and collaborate with physicians for quality, around‑the‑clock care.

We offer pay starting at $32.50–$34.50 per hour, plus experience-based increases, and a comprehensive benefits package including medical, vision, dental, a 401(k) with company match, tuition reimbursement, and generous paid

Qualifications

• LPN license in the state where employed.
• Minimum of one‑year experience in assisted living or other health care setting.
• Leadership skills and the ability to form relationships with residents, their loved ones and our co-workers.
• Ability to perform the physical tasks required in this type of role.

Responsibilities

• Form bonds with residents, their loved ones and physicians to create care plans using critical thinking and good judgment.
• Coach co-workers to monitor and support residents’ needs with urgency and responsiveness.
• Round with physicians, oversee orders and monitor medications to ensure quality care around the clock.
• Educate residents and families about health and wellness issues relevant to the care plan.
• Serve as and share the responsibilities of the Manager on Duty.
