Job summary
FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one, in-home and senior-living care in Davidsonville, MD, with optional telehealth options. You’ll evaluate, plan, and implement evidence-based treatment, driving meaningful improvement in mobility and independence for older adults.

Qualifications
- Licensed Physical Therapist in the state(s) of practice or eligible to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions to support wellness, mobility, and independence
- Document effectively within FOX’s EMR system
- Collaborate with an interdisciplinary team
- Educate patients, caregivers, and staff to promote safety and aging in place
- Provide telehealth/virtual care when appropriate
