The Care Team Home Health & Hospice in Mishawaka, IN is seeking a Registered Nurse Case Manager to join our hospice team. You will provide skilled nursing care to patients at home or in facility settings and coordinate with the interdisciplinary team for compassionate end-of-life support.

Hours are Monday–Friday, 8:00 AM–4:30 PM, with up to a 1-hour radius around Mishawaka; on-call may be required for additional pay.

Competenze

• Active RN license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation abilities
• Experience using HCHB (Homecare Homebase) preferred

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members

Overview

The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Territory: Up to a 1 hour radius of Mishawaka, IN
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Key Responsibilities

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
