Job summary

Senior Care Solutions is seeking a Home Health Registered Nurse in Boca Raton, FL to deliver skilled nursing care in patients’ homes, following physician orders and Medicare guidelines. You will work autonomously in the field with strong clinical support from our team.

Qualifications

• Active RN license in Florida required.
• 1 year Medicare home health experience preferred.
• OASIS experience preferred.
• CPR certification required.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services including medication management and wound care.
• Educate patients and caregivers to promote safety and independence.
• Accurately complete OASIS and clinical documentation in EMR.
