Job summary

Senior Care Solutions is seeking a Baylor Weekend Home Health Registered Nurse to provide skilled nursing visits in the Boca Raton to West Palm Beach region. You will work independently in patients' homes while receiving ongoing support from Clinical Managers and QA teams.

In this weekend role, you’ll perform SOC/ROC/Recertifications, medication management, wound care, and patient education, with a focus on safety, autonomy, and high-quality care within Medicare guidelines.

Qualifications

• Active RN license in Florida
• 1 year Medicare home health experience strongly preferred
• OASIS experience preferred
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services including medication management and wound care.
• Accurately complete OASIS and clinical documentation in the EMR on time.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Educate patients and caregivers to promote safety and independence.
