Job summary

Senior Care Solutions is hiring a Baylor Weekend Home Health Registered Nurse to deliver skilled nursing care to patients in their homes across the West Palm Beach to Delray Beach area, including Boca Raton. You will work independently in the field with ongoing support from Clinical Managers and QA teams.

You will perform SOC, ROC, and discharge visits, develop care plans, manage medications, wound care, and chronic disease management, while ensuring timely OASIS documentation and collaboration

Qualifications

• Active Florida RN license is required.
• 1 year Medicare home health experience strongly preferred.
• Experience with Wellsky/Kinnser preferred.
• OASIS experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Strong clinical assessment and time‑management skills.
• Comfortable working independently while staying connected to a team.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services, including medication management, wound care, patient education, and chronic disease management.
• Accurately complete OASIS and clinical documentation in our EMR in a timely manner.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Educate patients and caregivers to promote safety, independence, and improved outcomes.
• Adhere to all Medicare, ACHC, and agency policies and procedures.
