Job summary

Senior Care Solutions seeks a Registered Nurse (RN) – Medicare Home Health to deliver skilled nursing care to patients in their homes across Florida. You will perform SOC/ROC/recertification/discharge visits, follow physician orders and Medicare guidelines, and work independently with clinical support teams.

Ideal candidates hold a Florida RN license, at least 1 year Medicare home health experience, CPR, a valid driver’s license, and strong clinical assessment skills.

Qualifications

• Active Florida RN license is required.
• Minimum 1 year Medicare home health experience preferred.
• OASIS experience is preferred.
• Current CPR certification is required.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Strong clinical assessment and time-management skills.
• Comfortable working independently while staying connected to a team.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services including medication management, wound care, patient education, and chronic disease management.
• Accurately complete OASIS and clinical documentation in our EMR in a timely manner.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Educate patients and caregivers to promote safety, independence, and improved outcomes.
• Adhere to all Medicare, ACHC, and agency policies and procedures.
