Job summary

Senior Care Solutions is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes in Boca Raton, FL and surrounding areas. You’ll perform SOC/ROC/recertification, assessments, medication management, wound care, and patient education, while working independently with strong clinical support.

Qualifications

• Active RN license in Florida (required).
• 1 year Medicare home health experience strongly preferred.
• Experience with WellSky/Kinnser preferred.
• OASIS experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and active auto insurance.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services including medication management, wound care, patient education, and chronic disease management.
• Accurately complete OASIS and clinical documentation in our EMR in a timely manner.
• Collaborate with physicians, therapists, and interdisciplinary team members.
• Educate patients and caregivers to promote safety, independence, and improved outcomes.
