Job summary

Senior Care Solutions is seeking compassionate PRN Registered Nurses (RNs) to join our home health team in Florida. The RN will perform skilled nursing visits, start and resume care, recertifications, and coordinate with physicians and the interdisciplinary team to deliver high-quality patient-centered care.

You must hold an active Florida RN license and have at least 1 year of nursing experience; Home Health and Medicare experience, OASIS familiarity, and WellSky/Kinnser are a plus.

Qualifications

• Current, active Florida RN license.
• 1+ years of nursing experience.
• Home Health and Medicare experience preferred.
• OASIS experience preferred.
• Current BLS/CPR Certification.
• Valid Florida driver's license, reliable transportation, and current auto insurance.
• Excellent assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Experience with WellSky/Kinnser is a plus.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Routine Skilled Nursing visits.
• Complete comprehensive patient assessments and develop individualized plans of care.
• Administer medications, wound care, injections, IV therapy, catheter care, and other skilled nursing interventions as ordered.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Educate patients and caregivers on disease management, medications, home safety, and treatment plans.
• Maintain accurate, timely documentation in the electronic medical record.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure quality patient care.
• Ensure compliance with Medicare Conditions of Participation, agency policies, and state regulations.
