Job summary

Banner Health's Home Health division seeks an autonomous Registered Nurse to provide expert home-based care in the East Valley/Mesa area. You will assess patients in their homes, develop and implement individualized care plans, and coordinate with physicians and the healthcare team.

The role offers flexible scheduling and a supportive structure including a field preceptor, IT support, a dedicated scheduler and clinical leadership to help you deliver high-quality patient outcomes.

Qualifications

• Must possess a current RN license in state of practice.
• BLS certification required.
• Experience in home health or acute care settings preferred.

Responsibilities

• Assess patient physical, psychological, social and discharge needs.
• Formulate and update plan of care with patient and family input.
• Implement and supervise nursing care; coordinate with the care team.
• Document assessments and interventions clearly in the EMR.
• Promote continuity of care and patient education; participate in quality improvements.
