Banner Health's Home Health division seeks a Registered Nurse to join our clinical team delivering patient-centered home care in Mesa and surrounding West Valley communities.

As a Home Health RN, you’ll assess, plan, and implement nursing care with autonomy in patients' homes, coordinate with physicians and caregivers, and travel within designated territories. Per diem schedule offers flexibility, mileage reimbursement, and a company cell phone and tablet for documentation.

Qualifications

• Must possess a current, valid RN license in state of practice (Arizona).
• BLS certification is required.
• Experience in the clinical area is desired.
• Strong communication and collaboration with care teams is essential.

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate and revise a plan of care including discharge planning.
• Implement nursing care with physician orders and established care plans.
• Document assessment, planning, implementation and evaluation in the patient record.
• Mentor and precept other staff when appropriate.
• Promote quality care and patient outcomes in home health settings.
