Job summary

Home Sweet Home Health Agency is seeking compassionate Registered Nurses (RNs) to join our growing Home Health team across Maricopa County, AZ. This PRN (as-needed) role offers flexible scheduling and meaningful, one-on-one patient care in patients’ homes.

Ideal candidates are licensed RNs with at least 1 year of RN experience, preferably in home health or geriatric populations, and able to travel across East Valley, Scottsdale, and North Phoenix areas.

Qualifications

• Current Arizona Registered Nurse (RN) license in good standing.
• BSN preferred.
• Minimum of 1 year of RN experience required.
• Home Health nursing experience strongly preferred.
• Experience working with geriatric patient populations preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley, Scottsdale, and North Phoenix areas.
• Ability to work independently while delivering compassionate mobile care.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.
