Job summary

This role offers a structured team, mileage reimbursement, a company cell phone and tablet for documentation, with a schedule that includes day shifts and flexible weekly hours after training.

Qualifications

• Must possess current RN license in state of practice.
• BLS certification required; home health experience preferred.
• Experience in home health or ambulatory care is beneficial.

Responsibilities

• Assess patient needs and plan individualized home health care.
• Implement and coordinate nursing care in the patient’s home.
• Document and communicate patient progress to the care team.
• Provide patient and family education.
• Ensure compliance with policies and regulatory requirements.
