Job summary

Banner Home Health is seeking a Home Health Registered Nurse to provide autonomous care in patients' homes within the East Valley. You’ll plan, implement, and document nursing care, building one-on-one relationships with patients and families while managing a geographic caseload with moderate travel.

You’ll benefit from mileage reimbursement and the company providing a cell phone and tablet for documentation in our EMR system. Day shifts and training are offered to new nurses.

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in acute care is desirable.
• Arizona driver’s license and auto insurance may be required for Home Health.

Responsibilities

• Assess and plan patient care in the home setting with clinical autonomy.
• Implement and document nursing care per physician orders and policies.
• Coordinate with the care team and communicate patient status to caregivers.
• Evaluate patient outcomes and adjust care plans accordingly.
