Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings across Oklahoma. You’ll evaluate mobility, design care plans with physicians, and provide hands-on therapy to improve strength, balance, and independence.

This role offers autonomy, collaboration with nurses and therapists, and opportunities for ongoing clinical education. Must hold a DPT or Master’s in Physical Therapy and a current state license, with 1–2 years home health experience

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• 1–2 years home health experience preferred.
• CPR certification.

Responsibilities

• Evaluate functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans and safety.
