Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to deliver care in patients' homes and community settings in the Los Angeles area. You will assess function, create treatment plans, and help patients regain independence through real-life activities.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Current, unrestricted Physical Therapy license in the practicing state.
• CPR/BLS certification required.
• Minimum of one year of PT work experience preferred.
• Knowledge of neuro rehab techniques preferred.
• Ability to lift up to 100 pounds with safety compliance.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess progress and adjust plans.
• Conduct discharge evaluations and provide home safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to coordinate care.
