Job summary

UltaCare Medical seeks a Psychotherapist (LPC-MMSP/LCSW) to assess, diagnose and manage mental health in post-acute and long-term care facilities. You will work independently within nursing homes with flexible schedules and collaborate with nursing staff to optimize treatment.

Responsibilities include psychotherapy for geriatric residents with depression, anxiety, dementia and grief, plus documentation and coordination with an interdisciplinary team to improve quality of life.

Qualifications

• Unrestricted LCSW or LPC-MHSP license.
• Knowledge of standard documentation practices.
• Unrestricted licensure.
• Experience with geriatric populations and dementia preferred.
• Experience in post-acute or healthcare settings preferred.

Responsibilities

• Psychotherapy with people in skilled nursing, assisted living, and long term care settings.
• Deliver high-quality, patient-centered care rooted in best practices.
• Collaborate with an interdisciplinary team of medical and psychiatric providers to optimize quality-of-life and support facility.
• Support patients with depression, anxiety, adjustment disorders, dementia, grief, loss, and life transitions.
• Timely, accurate documentation.
