Job summary

UltaCare Medical has an immediate opening for a full-time Psychotherapist to assess, diagnose and manage mental health conditions in post-acute and long-term care facilities.

The role allows independent scheduling within nursing facilities, with collaboration from nursing staff and a focus on high-quality, patient-centered care for residents facing anxiety, depression, dementia and life transitions.

Qualifications

• Unrestricted LCSW or LPC-MHSP license.
• Knowledge of standard documentation practices.
• Experience working with geriatric population preferred.
• Experience working in post-acute or healthcare setting preferred.

Responsibilities

• Psychotherapy with residents in skilled nursing, assisted living, and long-term care settings.
• Deliver high-quality, patient-centered care rooted in best practices.
• Collaborate with an interdisciplinary team to optimize quality of life and support facilities.
• Support patients with depression, anxiety, adjustment disorders, dementia, grief, and life transitions.
• Timely, accurate documentation.
