Job summary

Humanity Plus Homes is seeking a Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities to live their best lives. You’ll build strong connections with individuals, families, host homes, and direct care workers, and coordinate personalized support in the community.

You’ll oversee service delivery, monitor provider relationships, conduct home visits for compliance, and provide on-call support.

Qualifications

A Bachelor’s degree in human services or related field with at least two years of related work experience.
Experience working with people with intellectual disabilities is a plus.
A valid driver’s license in good standing for home visits.

Responsibilities

Develop strong relationships with individuals, families, host homes, and direct care workers.
Help individuals express goals and ensure their voices are heard.
Monitor and optimize matches between individuals and home providers.
Collaborate to create Individual Support Plans aligned with policies.
Oversee service delivery and contract compliance through home visits.
Provide on-call support to providers and direct care workers.
