Job summary

Mosaic is seeking a compassionate Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities to live their best lives. You’ll build trusted relationships with the people we serve, their families, host home providers and direct care workers, ensuring advocacy and exceptional service day by day.

Qualifications

• Bachelor's degree in human services or related field required; minimum two years of related work experience.
• Previous experience working with people with intellectual disabilities is a plus.
• Valid driver’s license in good standing (home visits required).
• Strong communication, relationship-building, and problem-solving abilities; computer proficiency.
• Experience in behavior management and HCBS regulations preferred.

Responsibilities

• Build connections with individuals, families, host home providers and direct care workers; provide excellent customer service.
• Help individuals express goals and ensure their voices are respected.
• Monitor relationships between individuals and home providers to ensure a positive match.
• Collaborate with service planning team to create individualized support plans balancing wishes with policy.
• Oversee service delivery and contract compliance; perform home and environment visits.
• Provide on-call support to providers and direct care workers as needed.
