Job summary

Cascade Living is seeking an Associate Wellness Director LPN to provide medication management and skilled nursing care for residents in compliance with state regulations. The role supports a culture of wellness and purposeful living across our Grants Pass community and surrounding regions.

The ideal candidate will maintain an active LPN license, demonstrate compassion, communicate effectively with residents, families, and staff, and be able to handle on-call rotations and weekend shifts.

Qualifications

• Current state license as Licensed Practical Nurse (LPN).
• Compassionate individual representing the community professionally.
• Effective communicator with staff, residents and families.
• Ownership mindset to stay focused amid interruptions.
• Demonstrate good judgment, problem solving and decision-making skills.
• Ability to work flexible hours including evenings and weekends.

Responsibilities

• Maintain highest quality of life for residents and consistency of care.
• Implement residents’ personal care plans respecting preferences and routines.
• Assist in training and monitoring medication care administration.
• Demonstrate organizational skills for time management.
• Monitor caregiver performance to assure quality care.
• Educate residents and staff about health maintenance and disease prevention.

As anAssociate Wellness Director LPN (Licensed Practical Nurse), you’ll provide medication management and skilled nursing procedures for residents in compliance with state regulations.

$80,000-$90,000/annually
*Depending on Experience*

Job Type:

• Full Time
• 40 Hours weekly
• 5 days weekly
• Rotating On-Call and Weekend Availability Required
