Job summary

HopeWest in Grand Junction, Colorado is seeking an Associate Medical Director to lead clinical operations for 222+ PACE participants, overseeing their primary care needs and acting as the medical care expert.

Reporting to the Medical Director, you will supervise a team of NPs and collaborate with RNs, CNAs, PT/OT, managers, and the IDT, driving care plans, behavioral health initiatives, and ongoing clinical improvements.

Qualifications

• MD/DO licensed and board certified in Internal Medicine or Family Practice.
• Minimum of five years of clinical experience.
• Minimum of five years of supervisory experience preferred.
• PACE experience preferred but not required.
• Current DEA license (Controlled substance) or eligible.

Responsibilities

• Lead clinical operations for ~222+ PACE participants, overseeing primary care needs.
• Oversee a team of Nurse Practitioners and collaborate with RNs, CNAs, PT/OT, Managers and IDT.
• Drive ongoing clinical initiatives including care plans, behavioral health, and case reviews.
• Foster trust through strong staff relationships, assess team dynamics, and drive improvements.
• Manage workflow development, fall risk oversight, committee reviews, finances/invoices, and pharmacy workflows.
