Job summary

Sun Terrace Hermiston is seeking an Assistant Wellness Director to support the Wellness Director in coordinating health and supportive services at our Assisted Living community. You will oversee daily clinical operations, train and supervise staff, and ensure quality, proactive care for residents.

Candidates should be an LPN in good standing with at least 2 years in geriatric care and 1 year in supervision, including budgeting and scheduling.

Qualifications

• Current LPN in good standing.
• Minimum of 2 years in geriatric/senior care nursing with medication administration.
• One year in supervisory/management with budgets, staff development, training and scheduling.
• Ability to creatively problem-solve in resident care and employee management.

Responsibilities

• Foster resident-centered planning and delivery of services.
• Assess health, functional and psychosocial status and initiate individualized service plans.
• Coordinate with physicians, nurses, home health staff, case managers and state agencies.
• Arrange ancillary services (pharmacy, podiatry, lab, x-ray) as needed.
• Delegate nursing tasks within legal scope and communicate with families and practitioners.
• Provide training and supervision to staff to uphold the philosophy of care.
• Communicate health care needs with residents’ families.
• Provide instruction and reassurance to residents and families on health management.

Job description

We are Sun Terrace Hermiston, a retirement and assisted living community providing each individual compassionate care in a nurturing environment. We are looking for caring and compassionate individuals to join our team.

As an Assistant Wellness Director, you will assist the Wellness Director with the coordination and oversight of health and supportive services provided to residents at an Assisted Living Community. Responsible for the day-to-day clinical operations of the community to ensure resident’s healthcare needs are met. Provides training, supervision, and monitoring of associates in the administration of direct care within the community. Performs ongoing assessment/observation of residents’ physical and psycho-social needs and coordinates with other departments to assure quality, proactive care. Interacts directly with associates, residents, resident family members, vendors, and professional contacts within the community. This position reports to the Wellness Director.

You will:

• Foster a non-clinical, resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
• Assesses health, functional and psychosocial status of residents, initiating individualized service plans, proactively managing care and services for each resident, and maximizing the resident’s opportunity to remain in the assisted living environment.
• Ensures that residents’ care needs are met by coordinating with other community professionals, including physicians, discharge planners, nurses, home health agency staff, case managers, and state agency personnel.
• Arranges ancillary services, such as pharmacy, podiatry, lab, and x-ray as needed.
• Delegates nursing tasks as allowed under applicable nursing laws and practice acts, communicating regularly with residents, their families, and their health care practitioners.
• Provides training, supervision, monitoring of staff as they provide personal care, medication management, and other health-related services, assuring philosophy of care is practiced.
• Communicates regularly with residents’ families regarding health care needs.
• Provides instruction and reassurance for residents and families in maintaining health and managing medical conditions.

You currently:

• LPN in good standings.
• Ability to creatively problem-solve in both resident care and employee management
• Minimum of 2 years working in geriatric/senior care nursing, including experience with medication administration, delivery systems, and pharmacies.
• One year in supervisory/management position, with experience with budgets, staff development, training, and scheduling.
