Living Well Disability Services in Mendota Heights, MN is hiring for an Assistant Program Manager to support direct care and supervision across our care teams. The role focuses on coordinating services, maintaining daily living environments, and ensuring safety and quality of care for people served.

The position emphasizes leadership, shift supervision, scheduling, and proactive communication with families, case managers, and colleagues.

Qualifications

• Must be 18 years or older
• Valid Driver’s License
• Pass a background check through MN Department of Human Services
• Two years relevant post‑secondary education or equivalent experience preferred
• Technical skills – comfortable and proficient with computers/smart technology
• Prior leadership experience preferred and experience coordinating schedules
• Ability to lift and/or move up to 30 pounds regularly and 100 pounds occasionally

Responsibilities

• Provide direct and personal care, and support with activities of daily living to our people served
• Maintain a safe, clean, and comfortable environment; fix or report damaged items, maintain cleaning schedule, vehicle maintenance, meal planning, oversee medical equipment
• Manage staff by supervising shifts, providing input on performance reviews, coaching/disciplinary actions, leadership, training, and support
• Track individuals’ medical needs and appointment calendars, schedule appointments, accompany to appointments, order medications and medical supplies
• Engage in proactive communication with people served, coworkers, families, work/day programs, case managers and others
• Sign individuals up for activities, keep program calendar up to date, communicate with team
• Manage Person Centered Planning/CSSPA process and paperwork for people served
• Complete other tasks and responsibilities as assigned
