Responsibilities

• Partner with leadership to guide a team delivering person-centered support under 245D rules.
• Develop individualized service plans reflecting residents' needs and preferences.
• Ensure access to medical care and behavioral supports in a safe, positive environment.
• Maintain accurate service documentation and compliance with regulations.
• Coordinate transportation as needed with leadership for accessibility.
• Participate in quality assurance and ongoing service improvements.
• Provide training and coaching to staff to support growth and learning.
• Assist with scheduling and on-call coverage in collaboration with leadership.
• Perform other duties to support team goals and resident well-being.
