Qualifications

• Bachelor’s degree in human services or related field with 3 years of human service delivery experience required.
• Master’s degree in Social Work, Psychology, or related field preferred.
• Experience with populations such as trauma survivors, homelessness, substance use disorders is required.
• Experience with de‑escalation is required.

Responsibilities

• Ensure staff are entering data accurately and charts are complete with supporting documentation.
• Ensure staff compliance with time and attendance policy; enter edits and approve timecards when needed.
• Deliver case management and Life Coach services to a small caseload of residents.
• Review case records for documentation, data standards, and eligibility.
• Conduct monthly support groups to promote social, emotional, and health development.
• Train new hires on AWARDS and TMS (MAVEN).
• Perform data integrity checks and collaborate on corrective actions with the Program Director.
• Supervise case managers, Life Coaches, and Young Adult Peer mentors to meet intake and discharge goals.
• Identify community resources to meet individualized service goals.
• Develop relationships with DHS shelter staff and HRA for continuity of services.
• Complete and maintain all required internal and external reports.
• Monitor program performance for DHS/DOHMH/contract alignment.
• Oversee documentation standards and incident reporting.
• Track housing stability, engagement, and service utilization outcomes.
• Assist in recruitment, training and orientation of new staff.
• Participate in supervision, meetings, and trainings.
• On-Call availability for emergencies.
