Job summary

GEO Reentry Services LLC is hiring an Assistant Case Manager in California to support day reporting center operations. You will assist with intake, case coordination and documentation, using evidence-based practices and data entry within the company system.

Role includes group facilitation of CBT programs, client progress tracking, and collaboration with Case Managers. A strong service orientation, basic math, and bilingual capability may be required; ample training and growth opportunities are

Qualifications

  • High school diploma or equivalent required.
  • College coursework in Human Services or business preferred.
  • Bi-lingual English/Spanish may be required.
  • Solid computer skills including MS Word and Excel.
  • Strong customer service and communication abilities.

Responsibilities

  • Greet participants entering the facility and perform check-ins as directed.
  • Conduct drug screens per schedule and document results.
  • Complete intake processes and input data into the company system.
  • Provide general office administration and customer service to staff and participants.
  • Assist in case coordination and reporting with Case Managers.
  • Support in managing participant caseload and updating progress in the system.
  • Facilitate CBT programs and life skills groups; track attendance and progress.
  • Maintain documentation and prepare charts and reports as assigned.

Responsibilities

Summary


The Assistant Case Manager provides case management support for a Day Reporting Center checking in participants and assisting Case Managers with case coordination, case management, and documentation. Learns, uses and models the Principles of Effective Intervention (What Works) through evidence-based practices in all interactions with participants and staff members.


Primary Duties and Responsibilities



  • The Assistant Case Manager greets participants entering the facility. May complete drug screens according to assigned random schedule for each participant check-in.

  • May perform breath analysis on each participant who enters the facility for any purpose and provides a written or computerized result.

  • The Assistant Case Manager completes initial intake process with participants to completion, including intake paperwork, input into Company database and assignment of groups and other services.

  • Provides general office administration duties including answering phones. Provides general customer service to all who enter the facility, including participants, customers and the general public.

  • The Assistant Case Manager assists Case Managers with case coordination, communication with appropriate agencies and preparation and distribution of reports.

  • Assists in managing a participant caseload using the Principles of Effective Intervention (What Works). Conducts risk/need assessments and creates individual treatment plans for each assigned participant. Meets regularly with participants and documents participants’ progress or regress in Company database.

  • The Assistant Case Manager facilitates evidence-based CBT programs and other life skills programs to participants in groups each week. Documents participants’ attendance, participation and progress in Company database.

  • Develops and monitors the participant’s progress relative to their CMP through all phases of the program.

  • Files, prepares charts and performs data entry as assigned.

  • Responsible for office opening and closing procedures.

  • May be responsible for installing and removing electronic monitoring equipment on participants and forwarding documentation to the appropriate agencies and the Company monitoring center.

  • May clean electronic home monitoring (EHM) equipment.

  • Performs other duties as assigned.


Qualifications

Minimum Requirements



  • High school diploma or equivalent required.

  • College coursework in Human Services or business discipline preferred.

  • Minimum of six (6) months’ experience in Human Services field with direct participant services preferred.

  • Minimum of six (6) months experience working with minority populations preferred.

  • Must be twenty-one (21) years of age with at least five (5) years driving experience if required to transport participants.

  • Valid state driver’s license and five (5) year record of safe and competent driving as demonstrated by state motor vehicle report if required to transport participants.

  • Ability to safely operate a motor vehicle if required to transport participants.

  • Good customer service skills.

  • Effective communication skills with internal and external contacts at all levels.

  • Good organizational skills and attention to detail.

  • Solid computer skills and proficiency with MS Word and Excel.

  • Basic database skills.

  • Basic math skills.

  • Bi-lingual (English/Spanish) skills may be required.

  • Ability to work with computers and the necessary software typically used by the department.
