Job summary

GEO Reentry Services LLC in Chicago provides case management support for a Reentry Service Center. The Assistant Case Manager checks in participants, assists Case Managers, and supports staff with documentation and coordination.

This full-time role requires a high school diploma, customer-service skills, and experience in human services. It offers a $1,500 sign-on bonus paid in installments, a comprehensive benefits package, and opportunities to develop skills in evidence-based interventions.

Qualifications

  • A high school diploma or its equivalent.
  • Must be twenty-one (21) years of age with good customer services skills.
  • College coursework in Human Services or Business discipline preferred.
  • Minimum of six (6) months experience in Human Services field with direct participant services preferred.
  • Minimum of six (6) months experience working with under-served populations preferred.
  • Good organizational skills and attention to detail.
  • Solid computer skills and proficiency with MS Word and Excel. Basic database and math skills.
  • Bi-lingual (English/Spanish) skills may be required.
  • Effectively communicate with participants, program staff, corporate management, and agency personnel.
  • Logically express thoughts in English, in written and verbal form using correct grammar.
  • Ability to interact in a professional manner in both individual and group counseling settings Excellent oral and written communication skills.
  • Ability to work with a team of professionals and have the ability to implement team strategy approaches to work assignments.
  • Ability to work with computers and the necessary software typically used by the department.

Responsibilities

  • Checks-in all participants entering the facility and documents results within GEOTrack.
  • Performs breath analysis on each participant and documents results in GEOTrack/Company database.
  • Completes initial “Starting Point” process with participants and assigns groups and other services.
  • Provides general office administration duties and customer service to participants, customers, and the public.
  • Assists RSC staff with case coordination and preparation/distribution of reports.
  • Assists in managing a participant caseload and conducts risk/need assessments with treatment plans.
  • Facilitates evidence-based CBT programs and tracks attendance and progress.
  • Develops and monitors progress relative to Behavior Change Plans and Parole Board Orders.
  • Files, charts, and performs data entry; manages opening/closing procedures.

Responsibilities

Summary


The Assistant Case Manager provides case management support for a Reentry Service Center, checking in participants and assisting Case Managers, Substance Abuse Counselors, Employment/ Education Coordinators, and Supervising Case Managers with case coordination, case management, and documentation. Learns, uses and models the Principles of Effective Intervention (What Works) through evidence-based practices in all interactions with participants and staff members.


Primary Duties and Responsibilities



  • Checks-in all participants entering the facility. Complete drug screens according to assigned random schedule for each participant check-in and documents results within GEOTrack.

  • Performs breath analysis on each participant who enters the facility for any purpose, provides a written or computerized result, and documents within GEOTrack/Company database.

  • Completes initial “Starting Point” process with participants to completion, including Starting Point paperwork, input into Company database, and assignment of groups and other services.

  • Provides general office administration duties, including answering phones; provides general customer service to all who enter the facility, including participants, customers, and the general public.

  • Assists all RSC staff with case coordination, communication with appropriate official agencies, and preparation and distribution of reports.

  • Assists in managing a participant caseload using the Principles of Effective Intervention (What works). Conducts risk/need assessments and creates individual treatment plans for each assigned participant. Meets regularly with participants and documents participants’ progress or regress in GEOTrack/Company database.

  • Facilitates evidence-based CBT programs and other life skills programs to participants in groups each week. Documents participants’ attendance, participation and progress in GEOTrack/Company database.

  • Develops and monitors the participant’s progress relative to their Behavior Change Plan/Parole Board Orders through all phrases of the program.

  • Files, prepares charts, and performs data entry as assigned.

  • Responsible for office opening and closing procedures as directed by Supervisor.

  • Performs other duties as assigned.
