Job summary

Ascension Sacred Heart Pensacola is seeking a Case Management professional to support patient transitions and coordination in a full-time Day shift in Pensacola, FL.

How you’ll make an impact in this role

• Serve as a key liaison between case management, admissions, payers, and healthcare providers to streamline patient transitions and care coordination.
• Obtain and record service authorizations, process denial reporting, and document all payer interactions to keep relevant clinical staff updated.
• Facilitate, document, and transmit medical records and patient choice referrals to post-acute facilities or agencies to expedite safe patient discharges.
• Coordinate patient transfers and schedule ancillary services in strict accordance with case management direction and regulatory guidelines.
