Job summary

Ascension in Pensacola, FL is seeking a dedicated RN Case Manager to coordinate the interdisciplinary care plan from admission to discharge. You will advocate for patient resources, monitor level of care, and collaborate with the care transition team to ensure timely and efficient service.

The role requires active nursing licensure in Florida, with CCMC certification preferred. This is a full-time day shift in Case Management at Ascension Sacred Heart Pensacola.

Qualifications

• Licensed RN in Florida required or pending licensure at hire.
• CCMC certification preferred (ACM also accepted).

Responsibilities

How you’ll make an impact in this role

• Coordinate the overall interdisciplinary plan of care for patients from admission to discharge for the Case Management department by creating plan for care across the continuum. Integrate patient/family preferences and values. Advocate for resources and removal of barriers for the patient, collaborating with payers for benefit optimization and advocating for effective and efficient utilization of clinical resources
• Monitor patients' medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
• Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of care plan.
• Serve as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care. Act as a liaison between patient and post acute services.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
